Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

It should be called an All inclusive resort. my mom had the personal experience of being there for a month when I tell you, the staff is amazing. They’re not just staff their family members now the DON Miss has an open door policy. Anytime you need her while there my mom did everything from having a personal musician getting nails done every day bingo. This was my first experience with a nursing home, but I don’t think there’s too many to where you can call the staff and say hey I can’t make it up there to see my loved one and they send you a selfie.

[review] => This is the second place my daddy has been in. They are definitely better than the first place he was at.
We have had some issues, my dad has a stage 2-3 bed sore on his butt that has been there for weeks and looks bad. According to a doctor I know, my dad should be moved every couple of hours to a different side till his sore heals. The bed sore is looking alot better. We appreciate that.
The food sucks there. None of it looks like it's made fresh. The food is high carbs and low protein. It was discussed with my brother that high protein would help heal his bed sore, per a nurse that we both spoke too. He is supposed to be seeing a nutritionist to discuss this. We hopefully will see improvement.
The facility doesn't smell bad. It is a clean facility.
My dad's roomie makes the room uncomfortable for my dad by making it too warm and his tv blares all the time. Can't really speak to my dad on the phone, all you hear is the tv in the back ground. It's not alway easy carrying on a conversation with my dad because of the tv and I have heard that from the people that visit my dad.
I know there are no places that everything is going to be 100%, I get that but they could be better that what we have seen, especially since it is supposed to be a 5 star place. I spoke to my brother and our concerns are being addressed.

[review] => My parents were discharged from Macclenny Nursing and Rehab today. They were there for about 6 weeks. In that time the amazing PT and OT department took a bed ridden patient and diligently worked with her until she is able to walk with her walker. My dad has severe neuropathy which made him unstable on his feet. Bruce is wonderful. He worked motivating my dad building his strength and stamina. I can’t say enough good about Kari, Jennifer, Taylor, Bruce and the whole crew in the gym for genuinely caring about my parents and helping them achieve their goals.
Speaking of caring, that describes the nursing staff and CNA’s to a tee. I can’t say enough good about the efficiency of the nurses and their general concern for their patients. The CNA staff is patient and responsive to their patients. It would have been much easier to change my mom’s brief, but she wanted to go to the restroom. For much of her stay, moving Mom was a two person job, but they did it allowing her to continue being continent. I can honestly say we did not get this level of support and care in the other nursing facility we were in.
I am grateful for the people at Macclenny Nursing and Rehab from the top down and the bottom up. EVERYONE there cares and does their best to make sure their patients are in a safe, clean environment, respected and cared for. The management is responsive and works hard to ensure needs are met. We were there for rehabilitation, and I would not hesitate to go there again.

[review] => My husband is staying at the Macclenny Nursing and Rehab Center. He was able to be in a private room by the nurse's station. So, they've been very cooperative with us. The physical therapy was good, although he's finished with that now. So, he's transitioning now into long-term.
The staff is not trained to deal with dementia and Alzheimer's. I'm having to teach them how to deal with him. But then they've been very willing to listen to my suggestions. They have some crafts. They have karaoke. But we don't go to any of the activities because my husband can't deal with the crowd

[review] => My mom and dad were residents here over 8+ years ago and then my dad passed away in 2014 and my mom had Alzheimer’s and this nursing home decided they could not care for my mom any more so they called and found another nursing home to send her to and wanted me to move her, I told them that I was not able to move her and that they needed to get a medical transport vehicle to move her. After my mom made it to the new facility the administrator of the new location called me and told me that Macclenny Nursing and Rehab had put my mom in a taxi cab and sent her to the new facility. My mom having Alzheimer’s didn’t know or understand what was going on and she was non verbal and the taxi cab driver had told the new facility that my mom was hitting him and tried to bite him while he was trying to drive her to the new place.
It is some sorry people who would stoop that low to have a patient that was non verbal and had full blown Alzheimer’s transported by taxi cab.
DO NOT USE THIS PLACE IF YOU TRULY LOVE YOUR FAMILY MEMBER!!!

I am currently a resident of Macclenny Rehab due a severe spinal injury that requires a great deal of physical care which I am given with professionalism, kindness, dignity and respect. All members of the staff work physically and mentally taxing positions going above and beyond to care for their patients as if they were family. They have one of the most demanding jobs you can have, and one not many could do.

Her short term memory was horrible she didn't get the care as a alzheimers patient she should have and deserved. She was allowed to lock herself in a bathroom for an extended amount of time. When they realized she was missing after about an hr they realized she had locked herself in there and they had from what I understood only 3 personal had a key to the bathrooms. So by the time they finally got the door open they found my mom dead in there. Once again my mother had no other major health issues other then alzheimers. Needless to say I was shocked and devastated. I didn't put her in there to die after only being there 2wks. I was told this was the best thing for my mom and I because I have been her sole care giver for the past few yrs and I was worn out. So placing mom here was suppose to be the best thing for both of us. Her especially because she really needed some interaction from people her own age and oh how she loved being around people you see she was a Minister. My question is how did I take care of her for so long and she's there 2wks dies locked in a bathroom. Am beyond devastated and the guilt am feeling for putting her there is overwhelming. PLEASE think twice before putting your loved one here. My mother wasn't the only one that had been neglected during this same time frame..

Last complaint: They will allow you to go by their window to speak to the family in nursing home. The windows are screwed shut to allow 3 inches of space to speak and communicate through.

She called my sisters and I on her cell phone in the middle of the night and told us to dial 911. They had pulled her trache out cleaning it. She was rushed to the hospital in Maccleny. We rushed there from jacksonville, by the time we got there the hospital stabilized her and they had to life flight her back to Jax. Honestly... [review] => I would never allow a loved one to visit this facility, people you care about should not be made to suffer. If medical attention is needed, there must be an alternative compared to the traumatic experience that those close to me had to survive. Their patient was transported there in the evening and then Life Flighted out 8 hours later. The nighttime staff was unorganized, inattentive and clearly did not care about anything more than payment for showing up to work. This may be the only negative review but as of today only 17 positive reviews have been posted, only 17 people have been impressed enough give their opinion.

[review] => This place was something out of a horror movie. Elderly crying for help. The place was not clean. They brought my mother a used portable toilet that smelt like several day old urine. My mother laid in her own urine for an hour before help. She didn't receive her medication or proper treatment. While the nurse was cleaning her trach she was so rough and didn't know what she was doing she ripped the sutures in her trach and caused my mother hemorrhaging, no oxygen and a stress induced heart attack. My mother almost died under the care of this facility. Please get your family members and friends out of this place. They need to be shut down. Thankfully my mother survived. This place doesn't deserve a one star rating.

[review] => I've been at this facility since January 18, 2019 for rehabilitation after surgery. Since arrival, I have been treating with the most professional, caring, and respectful manners from the entire staff here. The nursing, CNA's, activities, laundry, and the kitchen do a great job and their best to accommodate any needs or request. My most amazing experience here has been the therapy department including Physical, Occupational, and Speech. I am very grateful for the rehab and treatment I am receiving and if I had to pick a facility in the future it would definitely be Macclenny Nursing and Rehab.

The residents gets baths once maybe twice a week. They let bandages sit for weeks on sores until they rot off. They never do their proper procedures or paperwork. There is always a problem with something there. I would never leave anyone there with those incompetent people. I wouldn't even leave a rock in their care!

[review] => It appears that some of the staff at this facility have their priorities out of order.The financial office being this department.We have a family member is need of a medical procedure but it can not be done until the needed paper work is completed so the funds can be distributed for payment while our family member waits in pain.Without the requested documentation treatment can not be given.Nobody calls with an update & then when I call the staff member has left early.If they the person in need of this procedure I assure you they would find it a Priority but since it does NOT affect their well being it is on the back burner it appears? They drugged her to the point of being in coma state for seven days and was transported to ER because of being unresponsive. They tried every was possible from day one to send her to another home and or Psych ward. My mom came in facility able to walk and make her on soup. She left barely able to walk after less than a month. The food was not very good. The facility was nice and the CNA's were nice and very helpful.

[review] => My husband was in Macclenny and I didn’t like it. As far as a nursing home facility, it was horrible. They were not very professional. They broke his toe, and they dropped him. Many things happened that should not have occurred. They didn’t give him his meds when they’re supposed to. It was a very, very poor facility. I would hate for anybody to go there at all. The rehab people were awesome. They were great. They were very good. However, I had trouble with the assistant nursing people. They were supposed to give him his meds. They were supposed to help him get in and out of his bed. It is about an hour and a half from my home, so it wasn’t like I could jump in the car and be there in 10 minutes. My husband would call me and he would be upset because nobody would come and help him go to the bathroom because he couldn’t walk. You got a very poor facility there as far as their staffing. Also he couldn’t eat what they provided. Food was always cold.

This facility should not be called rehabilitation and nursing home. It should be called an All inclusive resort. my mom had the personal experience of being there for a month when I tell you, the staff is amazing. They’re not just staff their family members now the DON Miss has an open door policy. Anytime you need her while there my mom did everything from having a personal musician getting nails done every day bingo. This was my first experience with a nursing home, but I don’t think there’s too many to where you can call the staff and say hey I can’t make it up there to see my loved one and they send you a selfie.

My brother admitted here a couple months ago in very poor shape. The nursing and therapy care have helped him progress much more than we originally expected and he is doing great. The staff and leadership are very responsive and caring.
